Spring Sport Physicals will be held on Saturday, February 4, 2012 at 10:00 a.m. in the locker rooms at Will C. Wood High School.  Cost is $15 cash only.  Parents need to fill out the front page of the Blue Physical form and the parent and student must read and sign the back of this form.  Parents will also need to fill out the first 3 pages of the Medical History form and sign that form as well.  The student will then bring both forms to the physical, along with $15 cash and give to the Doctors, so they can do the physical.  You can print out Physical forms by clicking here and Medical History forms by clicking here or they can be picked up in the front office.  You have to have your physical forms turned in before you can go out for a sport.

Concussion forms need to be signed by the athlete and their parents. This is a new law.  Click here for a concussion form  that you can print out or you can get this form from your coach or in the front office. 

If you get your physical by your own Doctor, it has to be done by a Doctor or Nurse Practitioner.  Vacaville Unified School District does not allow physicals to be done by a Chiropractor.  Get a copy of your physical and staple it to the Blue Physical form and turn it into the Athletic Director.  You still have to fill out the Blue Physical form and sign the back.
